The Scotts Valley / San Lorenzo Valley Soccer Club would greatly appreciate receiving your feedback to assist us in evaluating the performance of our coaching staff for the Fall 2009 season. This information will be used to assist us in making coaching appointments for the Spring/Fall 2010 season. Your comments and evaluation will be held in the strictest confidence.
