Required Forms - Trainer Registration

Where a team coach or manager engages a paid trainer to assist with the training of the team, the Club requests that the below procedures be followed. In general, where payments are made to independent contractor / trainers, the issuance of IRS Form 1099 to those trainers is required. In all cases, the trainer must complete an Team Official Registration & Disclosure Form (see link below) and a Form W-9 (see link below).

The Club requests that the following procedures be observed where a Division III Team engages the services of a paid trainer:

1) Prior to engaging the trainer, have the trainer complete an IRS Form W9 & Team Official Registration & Disclosure Form

(Click Here for Team Official Registration  & Disclosure Form)

2) Mail the completed W9 form to the Club Registrar, Beth Trenchard, at the Club's PO Box

3) Rather than the team or its representative making payments directly to the trainer, payments to the trainer should be made by the Club, through the account of the Division III team engaging the trainer for services

4) After the end of the calendar year in which payments are made (the January following the payment year), the Club will issue the trainer an IRS Form 1099, reflecting the amount of the payments made during the course of the calendar year to the trainer
