-SV/SLV Soccer Club Player Financial Assistance-
The SV/SLV Soccer Club (the Club) provides a limited amount of financial assistance available for the Fall season. In order to be considered for financial assistance, please note the following:
Application Deadline is June 1st
The application deadline to be considered for scholarship assistance is June 1st. Applications should be mailed with the completed player registration / membership form.
Demonstrated Financial Need
Financial assistance is awarded based upon demonstrated need. Applicants must complete an application and be prepared to show further evidence of need (see application) should the Board require it.
Financial Assistance is Awarded On First Come First Serve Basis
Financial assistance is awarded by the Club are awarded on a first come - first serve basis.
Packet Needs to Be Complete to Be Considered
If any information is incorrect or missing, the application will be denied. Financial assistance applications will be reviewed by the SV/SLV Soccer Club Board of Directors (the Board) and a decision will be made in an upcoming Board meeting. Partial Payments
The Club asks families receiving assistance to contribute at least $35 per player, but more if you can ($50, $75 or more). Please include payment with this application. Your partial payment will be returned if the Club cannot provide the assistance requested. APPLICATION PACKET CHECK LIST:

Fall Registration Form CYSA Registration Form – 2 COPIES CYSA form must be signed in a colored ink (Black ink is not accepted). Two copies of this form must be turned into your Club Registrar. Either two original forms --OR -- one original and one copy . Check for Partial Payment (minimum of $35 per player) Player Proof of Birth (if player did not submit birth certificate last year to Club) MAIL MATERIALS TO:
